Video Testimonial Release Form

I understand that my ebike Assembly or Testimonial Video ("Video"), will be shared with Really Good Ebikes ("RGE") via YouTube, and that said Video may be used promotional purposes. I authorize RGE to use my Video for such purposes.

I understand that I am not obligated to show my face in the video, but that an audio component will be required to present a meaningful testimonial or assembly video.

I hereby irrevocably authorize RGE to copy, exhibit, publish or distribute the Video for purposes of publicizing RGE products and services, or for any other lawful purpose.

The Video may be used on commercial websites or in any other distribution media. If the Video is used for commercial purposes, I will receive a one-time, $150 refund on my purchase, by the method of payment used for the purchase.

I agree that I will make no other monetary or other claim against RGE for the use of the Video.

I waive any right to inspect or approve any marketing materials which incorporate the Video, wherein my likeness or my testimonial appears.

I hereby hold harmless and release RGE from all claims, demands and causes of action which I, my heirs, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I have read the authorization and release information and give my consent for the use as indicated above.
